MFA Hardware Tokens - Instructions to Request Hardware Token
How to Request an MFA Hardware Token
1) Navigate to the UTS Contact Us page.
2) Click ‘Log a Service Ticket’ under ‘Self-Service’. Login with your MacID credentials when prompted.
3) Under ‘Request Types’ (shown on the left taskbar) click on IT Security > Multi Factor Authentication Support.
4) When the ticket creation window pop-up, under ‘My issue is related to…’, select ‘MFA Initial Setup’ in the first column and ‘requires hardware token’ in the second column.
5. Continue to fill out the rest of the form. When completed; click ‘Create’ and a ticket will be generated to the UTS team to fulfill your request.
